Hoops University 3v3 Team Registration

Total cost for this League is currently ~~$450~~ $299 per TEAM

Each TEAM only needs to be signed up ONCE.

PLAYERS do NOT need to register individually.

To complete registration, you will need the following for each player: player's name, email, phone number, address, and t-shirt size.

Note: Player 1 is the parent (or the legal guardian) signing up the team, and if you are the parent of Player 1, you will be the primary contact for all updates, game times, and last-minute changes communicated by Hoops University.

I understand that it is our responsibility to field a team each week as a courtesy to others, using subs if necessary. I understand that if we are unable to fill a team for any reason I must email grantham@hoopsuniversitytraining.com at least 24 hours ahead of game time.

Important Liability Information *

I understand that each player must bring a PRINTED completed copy of the Liability Agreement to the site of the game or they will NOT be allowed to partake. I understand that any substitutes must also provide a completed form to participate.

Refund policy: Refunds will be issued if requested by email before the registration deadline and will be reduced by a $10 administrative fee. No refunds will be issued after registration closes
